An activity log and an approval record answer different questions. Keep the exact proposed change, named approver and event times together. A successful API response cannot fill in missing approval evidence.

This authored record schema checks for a request, action, object, exact before and after, named approver, approval time and declared result. If supplied, execution time cannot precede approval. Identical before and after text is rejected.

The Model Context Protocol guidance supports human control of tool use; it does not mandate this record format. Populated fields do not prove identity, authorization, enforcement or an actual outcome. Review those in the real system.
